Welcome to Vitality SarcoHealth. By accessing or using our website, virtual, and in-person services, you agree to comply with and be bound by the following Terms of Service. If you do not agree with these terms, please do not use our services.
This Terms of Service is intended only to supplement but not to replace or supersede the terms and conditions of your other Agreements.
1. Scope of Services
We provide various health and wellness services focused on Sarco (muscle) Health. This includes but is not limited to physical therapy and sarcotherapy services; strength training coaching and classes; and, active aging education, public speaking, and consulting. Our services do not constitute medical advice, diagnosis, or treatment. Always consult a licensed healthcare professional for medical concerns.
2. Patient Responsibilities
- Provide accurate and complete medical history and information.
- Follow treatment plans as prescribed by healthcare professionals.
- Respect appointment scheduling and cancellation policies.
3. Privacy and Confidentiality
Your privacy is important to us. We adhere to HIPAA regulations and other applicable privacy laws. Please review our Privacy Policy Webpage for details on how we handle your personal health information.
4. Use of Website and Content
- The content on our website is for informational purposes only.
- Unauthorized use or reproduction of our materials is prohibited.
- We are not responsible for third-party content linked to our site.
5. Payment, Insurance and Fees
- Some of our services may be covered by insurance; we will make a reasonable attempt to verify your coverage initially (prior to your first visit). We use a third-party eligibility portal to verify your insurance benefits. Not all insurance companies share their benefits with third parties. If we cannot verify your insurance coverage using our systems, it is your responsibility to contact your insurance company to obtain your own coverage.
- Ensuring your insurance policy remains active, and your coverage remains consistent is the patient’s responsibility.
- Payment is due at the time of service unless prior arrangements are made.
- If you choose to utilize your insurance for services of ours that are covered by insurance, we require at least 24 hours of advance notice to change or cancel your appointment.
- We cannot bill your insurance for services that you cancel or do not show up for, but our expenses for this time remain constant. Therefore, we charge a $100 fee for all no-shows and any changes and cancellations within 24 hours of your scheduled visit, regardless of the reason.
- If you are regularly late for your scheduled appointments, we may charge you $25 for every 15 minutes you arrive late, not to exceed $100 per hour.
- We understand that sometimes things happen that are out of your control so we give all of our patients an annual courtesy credit of $100 that can be put toward any late fees, or late cancellation/no-show fees. Once this credit is exhausted, you are responsible for paying any fees incurred timely if you desire to continue to use our services.
- SarcoHealth classes are paid in advance and must be used weekly or they are forfeited. Memberships may be paused for up to 3 weeks per year with a one-week notice by completing and submitting our Pause/Freeze Request Form.
- We reserve the right to refuse service for lack of engagement and update pricing and billing policies.
6. Limitation of Liability
We are not liable for any direct, indirect, incidental, or consequential damages resulting from the use of our services or website.
7. Modifications to Terms
We may update these Terms of Service at any time. Continued use of our services constitutes acceptance of any modifications.
